The Center for REHABILITATION
When illness or injury gets you down, we’ll
help you back up again in The Center for Rehabilitation. Located in the Church Home, the Center provides
all the therapeutic services you need to return to
independent living.
Our staff of experienced therapists offers a full
range of physical, occupational and speech therapies,
all tailored to your individual needs.

OUR SPECIALTIES
We specialize in orthopedic, neurological and cardiac
rehabilitation services for those who have planned
surgeries and for those who suffer from sudden injuries
or illness.
While you are here, your care is entrusted to a
team that includes, in addition to the therapists,
medical and nursing staff, therapeutic-recreation
specialists, social workers, the chaplain and registered
dieticians.

ROSEWOOD REHAB
If you’re staying with us as a residential
rehabilitation patient, your home will be right around
the corner from The Center for Rehabilitation on Rosewood
Rehab. Here you will find your own private room and
bathroom, with all the comforts you want for your
short stay with us, including telephone and cable
television. Here you will also find a nursing staff
specially trained to assist you with your individual
treatment program.

YOUR ROOM IS READY
If you know you’ll need rehabilitation following
a planned surgery, call us ahead of time to arrange
a tour and get more information. If you find yourself
suddenly faced with the need for therapy, tell your
physician or care coordinator you want to come to
the Church Home. We’ll have your room waiting
for you!

ON YOUR WAY HOME
AGAIN
Before your discharge, our therapist may visit your
home to evaluate it for safety and determine what
equipment you’ll need to maximize your independence.
The therapist will provide you with a detailed written
report, including recommendations and modifications.

OUT-PATIENT SERVICES,
TOO
Outpatient occupational and physical therapy are available to you at The Center for Rehabilitation following discharge. Returning for therapy as an outpatient can help to maintain the continuity of your care.
